1. Are you a new client?
   • NO, I’m an existing Balanix client – please proceed to Step 2.
   • YES, I’m a new client to Balanix – please fill in this NEW CLIENT FORM and make sure you upload the completed form with your other documentation.
2. If you would like your TAX work completed, please fill in the relevant checklist below and upload the completed form with your other documents.
   • 2021 Individual Tax Return Check List
3. Complete the following form making sure you upload all your relevant documents (including any documents asked for in Steps 1 & 2 above)
